Details

The Intake Coordinator Supervisor plays a critical role in ensuring a smooth and compassionate admission process for patients and their families. This position supports the Director of Admissions in managing referral coordination, communication between internal teams, healthcare providers, and families. The ideal candidate will have a strong background in healthcare intake, excellent communication skills, and a passion for providing quality end-of-life care.

Qualifications

  • Minimum of 2 years in a healthcare intake, admissions, or case management role, preferably in hospice or home health
  • Strong leadership and team management abilities.
  • Proficient with Microsoft applications including Word and Excel
  • Strong time management and organization skills
  • Strong ability to multitask and prioritize
  • Excellent telephone and customer service skills
  • Proficient with medical terminology
  • Strong organizational skills and attention to detail.
  • Proficiency in electronic medical records (EMR) systems.
  • Complies with accepted professional standards and practice.
  • Demonstrates excellent observation, verbal and written communication skills
