We are seeking a LPN to work weekends (NOT REMOTE) to work both in the office and in the field seeing patients.  You will work 4-10’s Fri/Sat/Sun/Mon from either 8am-6pm or 9am-7pm. The duties will include 1.) intake 2.) LPN runner visits 3.) assist with operational duties on Friday and Monday i.e., phones, misc. duties as needed.

Details

The LPN Intake Coordinator is responsible for all aspects of the intake process. The intake coordinator will ensure the office obtains all necessary documentation, including orders, H&P, medication lists, and any other required information for admission. As an LPN, you will review records for eligibility of referrals and assessment of needs. Friday, Saturday, Sunday, Monday, 10-hour day shifts. Remote work with management approval, Fridays and Mondays will be in office days. Patient visits, field work, and marketing and in-service events will be required.

 

Responsibilities/Duties

1. Communicate information using current processes and technology available to the organization.

2. Process referrals, set up evaluations, and admits while communicating with the clinical team. Coordinate in-takes (See intake process sheet)

3. Informs and confers with DCS and ADCS on all patient, personnel, and service management needs and problems.

4. Manage the referral TT thread and create profiles and charts.

5. Develops an open, positive attitude and develops a positive rapport with all staff.

6. Assures the confidentiality of all patients, physicians, staff, and proprietary information.

7. Initial Chart audits

8. Update profiles for admissions and deaths in a timely manner

9. Audit and Upload consents

10. Assign the clinical team in Matrix

11. Answer phones when appropriate

12. Manage faxes- upload intake paperwork and referral faxes.

13. Update profiles for admissions and deaths in a timely manner

14. Order DME for patients

15. Cross-trained for LPN resource, including visits and field time as appropriate.

16. Any other duties assigned by the supervisor.

Qualifications

  • Current LPN licensure (in good standing) in the state(s) of practice with one year of experience under license
  • Current CPR Certification
  • Previous hospice experience preferred
  • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Work closely with patients on a 1-1 level but get strong support from a patient care team.
  • Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
  • Ability to learn an electronic medical records system.
  • Reliable Transportation, Driver's License, Car insurance
