Details

The Chaplain provides spiritual support, guidance, and resources to patients and their families.  They provide compassionate services to a diverse population encompassing all faiths that are represented by patients, families, and team members, while at the same time, reflecting the mission and philosophy of Suncrest.

Qualifications

  • Certificate of graduation from an accredited seminary or school of theology (BD, MDiv or equivalent theological degree), or a degree in pastoral care, or equivalent experience; Master’s degree preferred
  • One year of chaplain experience preferred
  • Experience working with death and the dying, as it relates to patients and their families/caregivers;
  • Ability to work as a contributing member of interdisciplinary group
  • Comfort level in inter-faith setting
  • Minimum of 3 units of CPE; and MDiv or Masters in Theology required.
  • Assesses spiritual needs of patients and families/caregivers in the hospice program and ensures that appropriate services are rendered in a timely manner;
  • Serves as liaison and support to community chaplains and spiritual counselors;
  • Maintains accurate records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision;
  • Attends patient care conferences and interdisciplinary group meetings, providing consultation, education, and support
  • Provides for funeral or memorial services for patients as requested;
  • Assists with the supervision of spiritual care volunteers when assigned to patients/families/caregivers;
  • Actively participates in quality assessment performance improvement teams and activities.
